RESUMO

Objective:To explore the effect of methamphetamine (MA) dependence on the attention of female youth, and to analyze the intervention effect of computer-based cognitive training on the attention of MA-dependent female youth.Method:From May to August 2021, a total of 64 MA-dependent female youths in abstinence period from a women's compulsory isolated drug rehabilitation center in Sichuan Province were selected as the MA group, and 53 ordinary female youths matched with their age and education level were selected as the normal control group.According to the matching principle of age, education level, and attention pre-test scores, the subjects in MA-dependent group were divided into MA-dependent intervention group( n=30)and MA-dependent waiting group( n=34). CogniPlus cognitive training system was used to train the attention of subjects in MA-dependent intervention group( n=30), while the subjects in MA-dependent waiting group( n=34) and normal control group did not receive training.The Vienna Test System was used to collect the attention scores of all subjects.SPSS 20.0 analysis software was used for data processing.Statistical analysis was performed using independent samples t test and paired samples t test. Results:The MA-dependent group had significantly longer endogenous alertness, temporary alertness, and concentration response time ((275.61±47.79)ms, (268.63±51.41)ms, (444.08±134.40)ms) compared with the normal control group ((247.02±34.09)ms, (237.60±46.04)ms, (355.15±44.37)ms) ( t=3.767, 3.405, 4.976; all P<0.05). After attention training, the post-test reaction time of endogenous alertness, temporary alertness, and concentration ((264.10±38.98)ms, (251.67±38.06)ms, (352.03±65.70)ms) in the MA-dependent intervention group were significantly shorter than those in the MA-dependent waiting group ((323.18±83.28)ms, (302.74±82.75)ms, (402.76±74.34)ms) ( t=-3.702, -3.232, -2.876; all P<0.05). The post-test reaction time of temporary alertness and concentration in the MA-dependent intervention group ((251.67±38.06)ms, (352.03±65.70)ms)were shorter than those of the pre-test ((265.70±37.84)ms, (428.67±120.11)ms) ( t=2.179, 3.588; both P<0.05). The MA-dependent waiting group had a longer post-test reaction time of endogenous alertness and temporary alertness(323.18±83.28)ms, (302.74±82.75)ms) compared with the pre-test ((285.35±51.43)ms, (271.21±61.42)ms) ( t=-2.752, -2.664; both P<0.05) and the post-test reaction time of concentration ((402.76±74.34)ms) was shorter than that of the pre-test ((457.68±146.29)ms)( t=2.431, P<0.05). The MA-dependent intervention group had longer endogenous alertness, temporary alertness, and pre-test reaction time of concentration ((264.57±41.41)ms, (265.70±37.84)ms, (428.67±120.11)ms)compared with the normal control group( t=2.083, 2.841, 3.230; all P<0.05). The post-test of endogenous alertness ((264.10±38.98)ms) was longer than that of the normal control group ( t=2.082, P<0.05). Conclusion:The cognitive training based on the CogniPlus system has a certain effect on the attention maintenance or recovery of MA-dependent female youth.It can be used as an intervention measure for cognitive impairment of drug addicts and help them healthy return to society.

Objective:To summarize the clinical experience of treating patients with severe acute organic fluorine poisoning using extracorporeal membrane oxygenation (ECMO).Methods:In January 2021, an acute mass organic fluorine gas poisoning incident occurred in Quzhou, Zhejiang Province. The clinical data of 4 severe patients with acute poisoning of organic fluorine treated by ECMO in our hospital were retrospectively analyzed, and the epidemiological characteristics, clinical symptoms, signs, the abnormal laboratory studies/examinations, and treatments of this kind poisoning patients, especially, the treatment pattern, support time, complications, and outcomes of ECMO were collected and analyzed.Results:All the 4 patients were male, with an average age of (52±9) years, and all of them came to the emergency department complaining chest tightness, cough and pharyngeal discomfort 6 h after exposure by inhalation. The patient’s condition progressed rapidly with severe acute respiratory failure and circulatory failure as the prominent manifestations. The mechanical ventilations were performed (13.0±4.8) h after poisoning, and ECMO treatment was performed (15.5±5.3) h after poisoning. Among them, 2 patients were treated using venoarterial (VA) ECMO, and 2 patients using venovenous (VV) ECMO, but 1 patient was converted to VA-ECMO 8 h later. The duration of ECMO support for the patients was (8.8±3.6) d. The duration of mechanical ventilation was (23.0±28.7) d and stay in intensive care unit was (42.0±55.4) d. Among them, one patient was transferred to a specialized rehabilitation hospital after the amputation surgery due to lower limb necrosis after VA-ECMO support, and the remaining 3 patients were discharged after recovery.Conclusions:ECMO support might have the irreplaceable value in the treatment of patients with severe acute organic fluorine poisoning, and should be considered as one of the reserves of regional health care system in dealing with public health emergencies.

Objective:To explore the characteristics of working memory ability of female methamphetamine dependent youths, and to analyze the effect of spatial and temporal training task (CODING) on working memory ability of female methamphetamine dependent youths.Methods:Sixty methamphetamine dependent youths from a women's drug rehabilitation center in Sichuan province were selected as the methamphetamine dependent group, while 60 female healthy volunteers from vocational high schools and universities participated as the healthy control group. Visual memory test (VISGED) was used to evaluat the working memory ability of the subjects, and SPSS 21.0 software was used for the nonparametric test of the memory difference between the two groups.The methamphetamine dependent group was divided into treatment group and waiting group based on the baseline matching principle of working memory. The treatment group was trained under the spatio-temporal CODING paradigm for working memory, and the training effect was assessed using the independent sample t-test. Results:(1)The individual parameters(0.06(-1.62, 1.67) vs 1.93 (-0.28, 2.71)) and working program(15.00(14.00 vs 16.00), 17.00(15.00, 20.00)) of working memory in methamphetamine dependent group and normal control group were statistically significant ( Z=-3.83, P<0.05, Z=-3.50; P<0.05). (2) Through memory training, the personal parameters before and after training in the treatment group ((0.08±1.79) vs 1.68 (0.68, 2.25)) had a statistically significant difference ( Z=-3.63, P<0.05). The personal parameters before and after the test in the waiting group ((-0.01±1.86) vs 0.72(-1.54, 1.88)) had no statistically significant difference ( Z=-1.35, P>0.05). Conclusion:Methamphetamine dependence impaire the working memory of female youths regardless of age or education level. Memory training can restore and improve memory ability to a certain extent, help to restore learning ability and living ability, and lay a foundation for a better return to society.

Objective:To screen the factors that were conducive to prolonging the effective half-life of 131I, and to provide a basis for selecting more suitable patients for the treatment of hyperthyroidism with lithium carbonate assisted radioactive 131I. Methods:Between July 2017 and March 2019, a total of 61 patients (23 males, 38 females, age: 13-73 (37.7±1.9) years) who received lithium carbonate adjuvant therapy before 131I treatment in the First Affiliated Hospital of Zhengzhou University were collected into this prospective study. Clinical parameters (serum free triiodothyronine (FT 3), serum free thyroxine (FT 4), iodine uptake rate (2 h, 12 h, 24 h) and ratio of iodine uptake rate (2 h/24 h, 4 h/24 h) before and after taking lithium carbonate were compared by using paired t test. The factors which were conducive to prolonging the effective half-life of 131I were analyzed by linear and multiple regression analyses. Results:Compared with the pretreatment results, lithium carbonate treatment significantly improved the 24 h iodine uptake rate ((74.82±2.69)% vs (82.38±2.33)%; t=2.674, P=0.010), decreased the ratio of 4 h/24 h iodine uptake rate ((88.96±2.85)% vs (82.12±2.27)%; t=2.644, P=0.010), prolonged the effective half-life of 131I ((123.03±3.09) vs (130.38±2.49) h; t=2.656, P=0.010), and decreased serum FT 3 ((26.46±1.65) vs (21.31±1.42) pmol/L; t=3.421, P=0.001) and serum FT 4 ((56.59±2.12) vs (49.24±2.19) pmol/L; t=3.289, P=0.002). Linear regression analysis showed that 2 h and 4 h iodine uptake rate, the ratios of 2 h/24 h and 4 h/24 h iodine uptake rate, and effective half-life of 131I before lithium carbonate treatment were related to the effective half-life of 131I after lithium carbonate treatment ( b values: from -0.576 to 0.642, all P<0.05). Multiple linear regression analysis showed that the ratio of 4 h/24 h iodine uptake rate was the main factor affecting the effective half-life of 131I after taking lithium carbonate ( b=0.642, 95% CI: 0.453-0.832, P<0.001). Multiple linear regression equation was y=-49.785+ 0.642 x. Lithium carbonate treatment could prolong the effective half-life of 131I if the ratio of 4 h/24 h iodine uptake exceeded 77.55%. Conclusion:The ratio of 4 h/24 h iodine uptake rate is the main factor affecting the change of the effective half-life of 131I after lithium carbonate treatment, and patients with hyperthyroidism whose ratio of 4 h/24 h iodine uptake rate exceeds 77.55% can effectively prolong the effective half-life of 131I after taking lithium carbonate.

Objective:To investigate the relationship between adenosine diphosphate (ADP) pathway-induced platelet dysfunction monitoring by thrombelastography with hospital mortality in patients with traumatic brain injury.Methods:The clinical data of 180 patients with traumatic brain injury in Zhejiang Quhua Hospital from January 2016 to December 2018 were retrospectively analyzed. The patients underwent thrombelastography examination. Among them, the ADP pathway-induced platelet inhibition rate (ADP inhibition rate) ≤ 60% was in 74 cases (non-ADP dysfunction group), and ADP inhibition rate > 60% was in 106 cases (ADP dysfunction group). Multiple Logistic regression analysis was used to analyze the independent influencing factors of patients′ hospital mortality. Logistic regression model was used to analyze the threshold of ADP inhibition rate to predict hospital mortality.Results:There were no statistical differences in the gender composition, age, prothrombin time, international standardized ratio, activated partial thromboplastin time, platelet count, systolic blood pressure, pulse, admission to thrombelastography examination time, Glasgow coma score, trauma severity score, simple trauma grading criteria and using of antiplatelet drugs before admission between 2 groups ( P>0.05). The intubation rate and in-hospital mortality in ADP dysfunction group were significantly higher than those in non-ADP dysfunction group: 69.8% (74/106) vs. 37.8% (28/74) and 32.1% (34/106) vs. 8.1% (6/74), and there were statistical differences ( P<0.01 or <0.05). The maximum amplitude and G value in ADP dysfunction group were significantly lower than those in non-ADP dysfunction group: (61 ± 9) mm vs. (65 ± 6) mm and (9 ± 4) kD/cm2 vs. (11 ± 3) kD/cm2, the ADP inhibition rate and arachidonic acid pathway-induced platelet inhibition rate were significantly higher than those in non-ADP dysfunction group: (76 ± 22)% vs. (45 ± 18)% and (75 ± 28)% vs. (35 ± 22)%, and there were statistical differences ( P<0.05). There were no statistical difference in the reaction time, blood clot formation time and angle between 2 groups ( P>0.05). Multiple Logistic regression analysis result showed that ADP inhibition rate >60% and trauma severity score were independent predictors of increased hospital mortality in patients with traumatic brain injury ( OR = 6.21 and 1.13, 95% CI 1.21 to 31.27 and 1.05 to 1.22, P<0.05). Logistic regression model analysis result showed that ADP inhibition rate >60% was the threshold for predicting the hospital mortality rate ( OR = 6.18, 95% CI 1.2 to 33.3). Conclusions:ADP inhibition rate of thrombelastography is related to the hospital mortality in patients with traumatic brain injury.

RESUMO

Cardiac surgery-related acute kidney injury (CSA-AKI) is a common and serious complication after cardiac surgery in adults. Currently, there is no specific examination method, and the diagnosis relying on serum creatinine and urine volume changes is of hysteresis. Biomarkers with the potential to predict CSA-AKI have become the focus in recent years. Clinical studies have shown that neutrophil gelatinase related lipid transporters and cell cycle inhibitors are of high diagnostic value; liver fatty acid binding protein can be used to assist in the diagnosis of CSA-AKI; microRNAs help to assess the poor prognosis of patients; the combined application of biomarkers may be used to predict the occurrence of CSA-AKI. CSA-AKI biomarkers provide the possibility for early clinical diagnosis and timely intervention, and are expected to become a new breakthrough in the diagnosis and treatment of CSA-AKI.

Objective To explore the characteristics and influencing factors of methamphetamine dependence adolescents’ emotional faces recognition ability. Methods Eighty-six adolescents methamphet-amine addicts and 85 healthy volunteers were tested by a self-designed questionnaire and an experiment of e-motional faces identification. Results (1)There was no significant correlation between the emotional faces recognition ability of the experimental group with their age,education level and drug time (P>0. 05). (2)The correct number of positive(32. 58 ±8. 56),neutral (32. 76±8. 06),and negative(56. 28±20. 04) emotional faces recognition in the experimental group was significantly lower than that in the control group of positive (35. 64±3. 91),neutral(35. 47±4. 00),and negative(71. 02±8. 62) emotional faces recognition(t=-3. 00,-2. 79,-6. 24,P<0. 05). Meanwhile,the response time of positive(( 401. 32± 175. 13) ms) and negative ((502. 08±194. 42)ms) emotional faces recognition in the experimental group was significantly lower than that in the control group of positive((300. 83±139. 48)ms) and negative((379. 91±197. 30)ms) emotional faces (t=4. 15,4. 08,P<0. 05). Nevertheless,there was no significant difference between the experimental group((400. 90±174. 21) ms) and the control group ((356. 67±156. 70) ms) in the recognition response time of neutral emotional faces (t=1. 75,P>0. 05). Conclusion Methamphetamine dependence impairs the ability of adolescents’ emotional faces recognition,and their positive and negative emotional recognition abili-ty has processing defects. The ability of methamphetamine dependence adolescents to emotional faces recogni- tion is not affected by their age,education level and time of drug uses.

Objective To analyze and explore the dynamic changes of postoperative serum TNF-α,IL-6,IgG,IgM and C3 levels in patients with craniocerebral injury,and to study the effect and evaluation on clinical significance.Methods Eighty-two patients who were diagnosed as craniocerebral injury and treated in our hospital from March 2015 to March 2017 were selected as the clinical research subjects,and the patients were randomly divided into the infection group and the non-infection group,41 cases in each group.The dynamic changes of postoperative serum TNF-α,IL-6,IgG,IgM and C3 levels of two groups were compared.Results Serum TNF-α and IL-6 levels at 1,3 and 7 days after operation in the infection group were higher than those in the non-infection group,and the differences were statistically significant(P<0.05).Serum IgM,IgG and C3 levels at 1,3 and 7 days after operation in the infection group were lower than those in the non-infection group,and the differences were statistically(P<0.05).At 7 days postoperatively,the serum IgG level in the infection group was restored to the levels in the non-infection group,and there was no significant difference(P>0.05).Conclusion Monitoring the postoperative serum TNF-α,IL-6,IgG,IgM and C3 levels in patients with craniocerebral injury is helpful for early diagnosis of postoperative infection,and has a high value in judging the severity of the disease.

Surgical site infection (SSI)following spinal surgery is the most common complications, which is devastating for both the patient and the surgeon.There is strong evidence in the literature that opti-mizing specific preoperative,intraoperative,and postoperative variables can significantly lower the risk of developing an SSI.Evidence-based approach will allow surgeons to minimize the risk of SSI and,therefore, significantly improve patient care.Here,we review the current evidence.

Pass an overview about medical social work ethics and analyzed the reasons.Based on China′s na-tional conditions and the basis of the particularity of medical social work in China, puts forward the paths of the lo-calization of medical social work ethics, namely, improve the adaptability of western medical ethics of social work, realizes the Chinese traditional culture and western medical social work ethics, mutual accommodation, with the so-cialist core values at the head of the medical social work ethic construction in our country.
